Penile Prosthesis, or Penile Implant, is a device that is inserted inside an individual's penis to achieve erection. The procedure is mainly addressed to patients who have failed to attain an erection after consuming oral medications and other conventional treatments. The benefits of Penile Implants include making your penis hard, similar to a natural erection, and eliminating the necessity for drugs.
Penile Prosthesis are of two types - Semirigid and Inflatable. Inflatable devices are further categorised into two, which are Three-piece Penile Implants that include two inflatable cylinders, a reservoir filled with salt water attached inside the abdomen or underneath the abdomen muscles, a pump at the scrotum, and a valve. The other is a Two-piece Inflatable Penile Implant that is comprised of an inflatable cylinder and a combined reservoir and pump. On the other hand, Semirigid Penile Implants, also known as Non-inflatable Penile Implants, use bendable rods made of silicon that can allow the individual to manually position the penis for sexual activity or adjust it to a flaccid state when needed.

Factors that make candidates eligible for Penile Prosthesis include:

During your initial consultation, a thorough evaluation based on your medical history and physical examination is done to confirm and determine the type of erectile dysfunction. Based on your assessment, treatment plans, and expectations are discussed, followed by a discussion on the type of implant required and its associated risks and benefits. You will be instructed on medications to avoid, fasting, and washing. You have to inform your specialist in case you have any past allergic reactions to anaesthesia or if you have had a urinary infection before. Moreover, you need to make the necessary arrangements for your transportation to and from the hospital, and for comfort items and clothes for your stay there.
Before the procedure, IV antibiotics will be administered, and the genital area will be shaved and washed with an alcohol-based antiseptic solution to reduce any infection. During the procedure, anaesthesia or moderate sedation is administered to mitigate pain and discomfort. A catheter is inserted into the bladder to allow the urine to flow out of your body. An incision is made in your genital area, and the implant is inserted into the corpora cavernosa, which are two columns of spongy tissue that help your penis achieve an erection by blood flow. If an inflatable implant is received, pumps and reservoirs are placed at appropriate positions, and the incisions are closed with absorbable stitches. You will be transferred to the recovery room after the procedure for further observation. The entire procedure may take around two to three hours, depending on the severity of your condition.
The mechanism of the Inflatable Penile Implants works by pressing the pump, which enables the saltwater in the reservoir to pass through the cylinders to achieve an erection with the help of valves. Later, the valve is released to drain the fluid back to the reservoir, returning the penis to a flaccid state. Three-piece Inflatable Penile Implants mimic and offer a natural and firmer erection and flaccidity compared to two-piece inflatable devices. However, the Three-piece Inflatable Penile Implants are prone to mechanical failure due to the number of components. Nevertheless, compared to Non-inflatable Implantable Devices, Two-piece Inflatable Penile Implants are mechanically complex.
Complications that may occur during Penile Prosthesis procedure include:

Cranial migration of the scrotal pump, which affects the sensation and appearance of the scrotum and penis.

Scarring
Penile shortening during the procedure, or due to trauma, post-priapism corporal fibrosis, or past treatment procedures like Radiation or Surgery
Device malfunction

Recovery time for a Penile Prosthesis varies from patient to patient. However, you should attain full recovery in around one to two weeks, although this can vary depending on the complexity of the procedure. Incisions may take up to five days to heal. It is normal to feel pain, soreness, or discomfort after the surgery, which may persist for two to three weeks and then gradually decrease. Penile Implants are usually activated two to four weeks after surgery, and specific instructions will be provided on how to operate the implant, including the processes of inflating and deflating it.
Yes, a Penile Prosthesis can be removed or replaced if complications arise or if the patient desires. However, removal may result in scarring or other changes to penile tissue.
