Seen in both children and adults, low platelet count or Thrombocytopenia is a condition where the person’s bone marrow is not making enough platelets. Platelets (thrombocytes) are the colourless blood cells that help blood clot, stopping bleeding in the person. Patients with low platelet count might bleed a lot, and bleeding is hard to stop. These platelets can be low in count due to bone marrow disorders such as leukaemia. Patients with certain medical conditions and certain medications have low platelet counts in common.
Thrombocytopenia Stages
The regular and healthy platelet count can range from 150,000 to 450,000 platelets per microlitre of blood in adults. However, in Thrombocytopenia condition, there are three stages based on severity level such as:
Mild Thrombocytopenia
Here, platelet count ranges from 101,000 and 140,000 per microlitre of blood.
Moderate Thrombocytopenia
51,000 and 100,000 platelets per microliter of blood can be seen in mild conditions.
Severe Thrombocytopenia
Here, the patient is at high risk of having only 51,000 to 21,000 platelets per microliter of blood.
Symptoms
The low platelet count symptoms are so mild that people may not realise they have Thrombocytopenia. The most common symptom is a nosebleed that is difficult to stop. Other common low platelet symptoms are as follows:
Blood in vomit
Blood in stools
Blood in urine
Bleeding gums
Heavy menstrual bleeding
Bruises
Petechiae (rash of reddish-purple spots on lower legs)
Purpura (excessive bruising)
Rectal bleeding
Causes
Platelet supply can be low when the bone marrow does not produce enough platelets or when your spleen traps platelets so they cannot circulate through the blood. In some cases, your body might produce enough platelets. However, you might have medical conditions such as TPP, bacteria in the blood, Haemolytic uremic syndrome, etc., that are using up or destroying your platelet supply. Other causes resulting in low platelet supply in your body might be due to:
Blood cancers
Cancer treatments
Autoimmune diseases that attack the immune system
Infections
Exposure to toxic chemicals
Certain medications
TPP (Thrombotic thrombocytopenic Purpura)
Alcohol usage
Diagnosis
On the identification of any of the above symptoms, you have to immediately seek medical attention. After conducting the physical examination and detecting bruises, rashes, and related symptoms, a proper diagnosis is performed by doctors based on medical history and chronic conditions. The various tests performed as part of diagnostic solutions to provide the right course of treatment include:
Complete Blood Count (CBC) Test
The most common test to check platelet counts and white and red blood cell levels. Consult with our experts to have the best treatment.
Blood Clot Test
PTT (partial thromboplastin time) and PT (prothrombin time) tests measure the time the blood takes to clot.
Peripheral Blood Smear
The examination of platelets under a microscope.
Bone Marrow Biopsy
The collection and examination of bone marrow.
Treatment
Treatment is often not required to improve the low platelet count. However, when the low platelets are causing severe issues, these treatments might be adopted such as:
Blood Transfusion
Transfusions can boost platelet levels for bout three days.
Splenectomy
Surgery to remove the spleen when your spleen is trapping many platelets. Your body can survive without a spleen since the liver takes over most of the spleen’s functions. Certain vaccinations are given to prevent infections in the absence of the spleen. Visit our internal medicine hospital in Mysore for the best treatment.
Steroids
Steroids often help with low platelet count.
