The blood cells are made in the bone marrow of humans. If malformed or less in number, these cells could not function correctly. This could be due to various diseases or conditions. In these cases, a bone marrow transplant (BMT) becomes valuable. In a BMT, unhealthy or abnormal blood-forming cells (stem cells) in the bone marrow are replaced with healthy ones. It is a non-surgical procedure as the new cells are injected into the bloodstream through an intravenous (IV) catheter or tube, similar to how blood or medicines are injected. Also known as hematopoietic stem cell transplant, this procedure is required for patients whose bone marrow stops working and does not produce enough healthy blood cells. BMT is usually done either after radiation and chemotherapy or both in children who need this procedure for whatever reason. Depending on the source of stem cells, there are two types of bone marrow transplants:

  • Autologous transplant (cells sourced from the patient themself)

  • Allogeneic transplant (cells are sourced from a donor, related or unrelated)

  • Autologous Transplant

When the stem cells for transplant come from the patient themself, it is called an autologous transplant. The stem cells are collected from the bone marrow or blood of the patient well before the transplant starts and then reused once chemo-immunotherapy (conditioning) for that transplant has been given. The autologous transplant helps by killing the cancer cells but retaining the body’s ability to produce the required blood cells and fight infections. Few experts call it high-dose chemotherapy followed by stem cell rescue.

  • Allogeneic Transplant

When the stem cells for transplant come from a healthy donor (related or unrelated), it is called an allogeneic transplant. In cases of leukaemia or other malignancies, the new stem cells from a donor destroy the cancer cells and remain in the body throughout life, thereby treating cancer. The donor's blood proteins, human leukocyte antigens (HLA), must match with patients for a donation to be possible. Earlier, HLA was supposed to be 100% matching for a transplant possibility. Still, nowadays, in related donors (donors from the family), even if HLA is just 50% matched, we can do an allogeneic BMT, popularly called a haploidentical bone marrow transplant. This option is for those patients who cannot find a suitable donor (in the family or the registries); we offer haploidentical bone marrow transplants. These transplants can be T-replete (post-transplant cyclophosphamide) or T-deplete (TCR αβ and B depleted) bone marrow transplants, depending upon the disease and condition of the patient. In haploidentical BMT, the donor is usually a family member (sibling or either of the parent), and we very rarely need someone like a sibling of a parent or even grandparents. The option of a haploidentical bone marrow transplant almost always ensures that they can be given one if a patient needs an allogeneic transplant. With time, the results of haploidentical BMT have matured and are almost the same as that of other allogeneic transplants.
