Hepato-Pancreatico-Biliary (HPB) surgery is a surgical procedure for malignant and benign diseases of the pancreas, biliary tree, and liver. A biliary tree comprises the bile ducts and gallbladder. The liver, pancreas, and the biliary tree, together, are responsible for more than 3600 functions within the human body. Thus, it is absolutely necessary to eradicate all the problems that arise in this organ system. HPB surgeries can be quite complex and require expert training experience and/or training.
Why is the procedure performed?
Following are the conditions that may require HPB surgery,
Primary Liver Cancer
Removal of Liver Tumour: The most common type of liver tumour to metastasize in the liver is known as colorectal cancer, which can be treated only post-surgical removal of the disease. Another metastasizing tumour in the liver is known as the neuroendocrine tumour in the gastrointestinal tract.
Malignant and Benign Diseases in the Pancreas: Tumours in the pancreas are quite common. However, the growth of neuroendocrine tumours in the pancreas and cystic tumours are also quite common.
Surgical removal of pancreatic malignancies can help cure and treat all the affected patients.
Diagnosis and Pre-procedure
The diagnosis for each of these diseases is different. When performing an HPB surgery, the following medical tests are conducted for the respective diseases:
Liver Disease: Ultrasound, Liver-function tests
Advanced Liver Disease: Fibroscan Or Acoustic Radiation Force Impulse Imaging (ARFI)
Liver Cancer: MRI, CT scan, Tumour markers
Acute Pancreatitis: S. Amylose, CT scan, ERCP
Chronic Pancreatic Disease: Endoscopic Retrograde Cholangio pancreatography (ERCP), CT scan.
Pancreatic Cancer: ERCP, CT scan, Tumour markers, Endoscopic Ultrasound (EUS).
Bile Duct Obstruction: ERCP, Magnetic Resonance Cholangiopancreatography (MRCP), EUS, Ultrasonography
Bile Duct Cancer: MRCP, ERCP, EUS, CT scan
Procedures
Complex hepatic (liver), pancreatic, and biliary surgeries, include some of the following procedures but are not limited to these -
Liver resection (major and minor)
Liver tumour ablation
Pancreas resection for benign and malignant disease (including Whipple procedure)
Bile duct resection
Bile duct repair
Complex gallbladder resection
When necessary, all of these surgical procedures (including major pancreatic operations and the Whipple procedure) can be carried out minimally invasively using laparoscopic or robotic surgical methods.
More about HPB surgeries
When a tumour is present at the head or the top of the pancreas, a Whipple resection procedure and a reconstruction of the blood vessel reconstruction may be recommended to remove tumours present in the tail of the pancreas as well as the body. This surgical procedure is called a distal pancreatectomy. HPB Surgeries in Yeshwanthpur, Bangalore is available at Manipal Hospitals, visit today.
In most cases, patients might have to get their bile ducts reconstructed completely. This entirely depends on how much damage the tumour has made up until that point and if the tumour is causing any blockage of the bile duct.
HPB surgeries also help with the treatment of Gallstones (small particles that get stuck in the gallbladder).
After procedure
In almost all cases, an ERAS (Enhanced Recovery After Surgery) program is initiated during the recovery period. This programme is said to be a framework that is designed at the patient’s preoperative stages so that they do not suffer bouts of pain and misery right post-surgery.
